Vital Records Project

We provide free legal assistance to low-income New Yorkers in the five boroughs, with the following issues related to Vital Records:

• Application to the NYC Department of Vital Records for birth/death certificate corrections
• Article78 proceeding to correct birth and death certificates
• Name Change
• Gender/Sex Designation Change

To apply for our services, you must complete an online application (please state in your application that you need help with one of the above-mentioned issues, such as a name change.) Our staff will get back to you within 3 business days, at which time you will be asked to send us documentation about your issue.
